TERMS AND CONDITIONS

 
  1. Definition
    Aashirwad is a healthcare plan from Apollo Hospitals, to provide health care service to the elderly. The service can be availed by any individual who is 21years or above.
      

  2. Availability
    The service is available in Apollo Hospitals at the following centers
     
    Delhi         -   Indraprastha Apollo Hospitals, Sarita Vihar, Delhi-Mathura Road, New Delhi

    Chennai    -   Apollo Hospitals, 21 Greams Lane, Off Greams Road, Chennai.

  3. Applicability

    1. The person who buys Aashirwad is called the Subscriber and the person who avails it is called the beneficiary. The subscriber can transfer this service only at one instance to any individual before any of the services of the Aashirwad Package are availed. 

    2. The beneficiary can avail all the services of Aashirwad Package only at the center selected at the time of purchase by the subscriber and he is not allowed to avail the services under this plan at any other centers where the Aashirwad Service is available.

  4. Service Tenure
    The service is available for one year from the date it is bought /subscribed for. 
     

  5. Product Description
    The Aashirwad service offers primary care medical facilities to its beneficiaries. The service consists of the following
    1. Master Health Check:
      The Apollo Master Health Check is a through test which is done once only during the tenure of Aashirwad and it provides extensive and detailed assessment of a person's present state of health. The check includes a medical questionnaire, full physical examination and a consultation with a doctor. Once the Subscriber buys the service for his/her beneficiary, the Aashirwad Coordinators contact the beneficiary and fix up an appointment for the beneficiary to undergo a Master Health Check. 

      The components of the treatment may vary from place to place and person to person and is dependent on the hospital, doctor and patient.

      If the beneficiary does not make himself available at the hospital as per the schedule/appointment for his Master Health Check, the same shall be considered as done and shall not be carried over.
       

    2. Friendly Call from Apollo every week 
      A health enquiry telephone call will be made to the beneficiary's residence every week at anytime between 9 AM to 5 P.M on the 1st day of every week i.e. on Monday or on such other days convenient to Apollo Hospitals. However, if the beneficiary is not available when such a call is made or if the call is disrupted due to faulty line or the phone being faulty or non functioning of the beneficiary's telephone then Apollo Hospitals shall not be held liable and such calls shall be deemed to have been made and shall not be carried forward to the next day/week.
       

    3. Bi-Monthly Home Visit
      Doctor from Apollo Hospitals would visit the beneficiary's residence to examine the beneficiary's general health condition. 

      The Apollo Doctor shall make a prior telephonic call 48hrs in advance of his/her visit and shall visit the beneficiary's residence at an address that is specified when buying the product or at the address last communicated to Apollo Health Street Limited.

      The beneficiary is responsible for ensuring that he/she is present with proper identification at the address notified. 

      If for any reason the Doctor is unable to locate the address notified or cannot approach the place or finds the door being locked or the beneficiary not being present or the doctor not being permitted entrance, it shall for all purposes be deemed that the monthly call has been done and the same call shall not be carried forward.
       

    4. Half Yearly Quick Health Check
      This test is performed only once, after six months and within a period of one year from the date of Master Health 
      Check test. The Quick Health Check is a small package of essential tests that reviews the beneficiary's condition and ensures that he/she continues to enjoy good health.

  6. COST
    The subscriber shall be charged an amount of Rs 7500 (USD 150 or equivalent of IRS 7500) per person per annum. This charge is liable for change at the discretion of APOLLO HEALTH STREET LIMITED from time to time. Any form of medication shall be at an extra cost, which shall be so decided by APOLLO HEALTH STREET LIMITED on a case-to-case basis. The charges once paid shall not be refunded.
     

  7. Payment Procedure:

    1. The subscriber should click on "Click here to gift the service". This link leads him to the page where he enters his and his beneficiary's details. Clicking on "submit" he is taken to the Payment gateway page, where he needs to enter his credit card number. Upon successful completion of the transaction the details of the subscriber and the beneficiary are sent to the Aashirwad Management Team. The Management Team Creates pages for both and sends them their respective login ids and passwords.

    2. Use of Credit Card: The subscriber is prohibited from making payment from the credit card of any other person and shall make payment due to APOLLO HEALTH STREET LIMITED through his credit card only.

  8. Acceptance:
    When once the Subscriber makes payment he/she is deemed to have accepted these terms and conditions and executed an agreement with APOLLO HEALTH STREET LIMITED.
     

  9. Customer Support & feedback: 
    The Aashirwad Help Desk promptly responds to Queries/Suggestions/feedback from the Customers. The customers can post their queries to aashirwad@apollolife.com. Customers can also reach the Customer Support Division on Phone No.040-3607777 - Extn: 6340 during the working days, Monday to Saturday from 9.00 AM (ISD) to 5.30 PM.(ISD)
     

  10. Indemnity 
    The subscriber/beneficiary shall indemnify APOLLO HEALTH STREET LIMITED from any losses or damages caused to or expenses incurred on account of improper use of credit card or breach of these terms and conditions by him.
     

  11. Termination 
    1. If for any reason, the subscriber/beneficiary desires to stop availing the service he/she may do so in which case he/she is not entitled to any refund of charges paid by him/her.

    2. The APOLLO HEALTH STREET LIMITED may terminate the access of the subscriber/beneficiary to the product and services if the subscriber/beneficiary fails to comply with these terms and conditions or guidelines notified by APOLLO HEALTH STREET LIMITED hereafter from time to time.

    3. Aashirwad Service cannot be transferred when it is in usage from one beneficiary to other.

  12. Change of terms and conditions 
    The APOLLO HEALTH STREET LIMITED reserves the right to change/modify/alter/reduce/modify the services offered and do all amendments in its terms and conditions as it deem fit at its discretion from time to time.
     

  13. Disputes redressal
    Any dispute arising out of these terms and conditions shall be within the jurisdiction of courts in Hyderabad, Andhra Pradesh, India.
     

  14. Service is personal:
    The service envisaged under this scheme is exclusively intended for the benefit of the Subscriber/Beneficiary as the case may be and such subscriber/beneficiary is provided with the services stipulated in condition 5 above and nothing more. This scheme does not apply to any person other than the Subscriber/Beneficiary.
     

  15. Liability
    The APOLLO HEALTH STREET LIMITED is not in any way liable for any expenses or losses incurred by the subscriber/beneficiary for any reason whatsoever in respect of the services under this Aashirwad Plan.
